Sagebrook Health Center specializes in in-patient rehabilitation with a primary goal of returning our residents to the highest level of independence. We have our own staff of physical, occupational, and speech therapists that have worked together with excellent results for several years. We report here the percentage of admissions by diagnostic category that have discharged to an independent residence or modified independent setting such as an Assisted Living Facility. Orthopedic diagnoses constitute our most prevalent category or approximately 50% of our in-patient rehabilitation residents.

We have broken this category down into more specific diagnostic groups in order to further detail our successful outcomes. We are extremely proud of our 100% return to independence rate on all elective orthopedic procedures such as joint replacements and spinal surgery.
