FAQs

Question – How do I pay?

Answer – This Health Center accepts Medicare, Medicaid, Private Pay and some insurance programs. For more information on how you can qualify for these programs, please call the referral hotline at 512-694-6093

Question – Will my loved one get Rehab or Therapy?

Answer – We offer three different types of therapy: Physical, Occupational, and Speech Therapy. Therapy is offered daily of each week. All therapy staff are employees of our Health Center and not contract therapists. If a new resident has physician orders for therapy they are assessed by one of the therapists. A plan of care and tentative discharge dates are set and the therapy begins.

Question – What are visiting hours?

Answer – There is 24-hour access to this Health Center. We do ask that if families visit after 10pm please check in at the Nurses’ Station.

Question – Tell me about your physicians?

Answer – A team of Board Certified Physicians from Austin Geriatric Specialists is on-site Monday through Friday through the Asista family of Health Centers. The AGS practice includes advance practice nurses who are trained and certified in Geriatrics. The AGS physicians manage the medical problems of each patient along with managing the Interdisciplinary Team that provides daily nursing and rehab care.

Question – What do I do to be admitted?

Answer – First, we encourage you to visit our Health Center and speak with the Admissions Coordinator. The Admission Coordinator can also help make the transition form the hospital or home into either the rehab or long term care easier. Once you decide, you then inform the case manager at the hospital. The case manager will call the Admissions Coordinator to discuss discharge plans for the resident. An onsite assessment will be done by our staff to determine if our Health Center can meet the resident's needs. If accepted, a discharge date from the hospital will be set. The hospital physician will write discharge orders which list all medications and care required for the customer. This process ensures a continuum of care.

Question – What do I bring?

Answer – If you are coming in as a rehab patient you will need to bring clothing that gives you plenty of room to move and be comfortable while in therapy. We encourage all potential residents to bring shoes that have slip-resistant soles. You are also welcome to bring pictures and personal items to make your stay more comfortable. We provide all toiletries and linens but you can bring your own if you choose. We do ask that any personal items be labeled with the resident's name.

Question – Will someone help us with the discharge planning?

Answer – Each week the therapists, social worker and nursing department get together as a team to discuss how each rehab patient is doing in therapy. The social worker will call the responsible party to inform them of the progress and the recommendations from the therapists. If the resident is able to return to home or assisted living, the social worker will make sure the resident is set up with Home Health Services, any equipment and prescriptions for medications that will be needed. If the resident is unable to return to the community, the social worker will then assist the family with long term placement either here or at another facility.
