Activities Program  The Activity department at Sagebrook is lead by Cindy Martinez and is tailored to reflect the interests and desires of our customers. All activities are designed to stimulate and challenge the mental and physical abilities of each client. Daily activities include reminiscing groups, musical entertainment, daily exercise, group outings, spiritual programs along with a variety of individual programs. All customers are encouraged to participate in some aspects of the activities program and are always included in any special events held at the facility. Sagebrook will also hold a Resident Council meeting on a monthly basis. These meetings are run by the residents, with the assistance of the Activity Director. Family members and loved ones are encouraged and welcome to attend any activity with the residents at Sagebrook. Volunteer opportunities are always available to those interested.

Social Services  Having a social worker on staff promotes family involvement and support of the residents, as our goal is to know our family members as well as each of our residents. A family support group meets each month to offer information and encouragement. These meetings are run by the family members with the assistance of the Social Worker. With her many resources and contacts within the field of long term care, Gretchen Golladay will be an integral part of your experience at Sagebrook Health Center.

Care Plan Meetings  Residents and families are encouraged to attend interdisciplinary care plan meetings. These meetings facilitate interaction between the various medical-care departments and provide a formal means of communicating with one another about the care of each customer. At a minimum, care plan meetings are held quarterly or more frequently when a resident's care needs change.

Dining and Nutrition  Our Dietary department is lead by Patricia Ruthstrom, FSS. Patricia comes to Sagebrook with 21 years experience in dietary services. Patricia's knowledge of providing quality food service within the long term care guidelines provides our residents with meals that meet their dining expectations. With the assistance of our consultant dietician, Sagebrook is able to meet all the dietary needs of our residents including, specialized diets, mechanically altered foods and liquids, and all other physician-ordered dietary requests. The dietary staff at Sagebrook offers residents a revolving menu of items and serves three meals each day.
